Who can take part

  • You are 18 years or older
  • You have been diagnosed with heart failure that causes symptoms (like shortness of breath or fatigue)
  • You have had a heart failure-related hospital stay or urgent care visit in the last 6 months
  • Your kidney function (eGFR) is between 20 and 60 (moderately to severely reduced)
  • Your blood NT-proBNP level is above 300 (or above 600 if you have an irregular heart rhythm called atrial fibrillation)
  • You are not currently taking a medication known as an MRA (like spironolactone or eplerenone)
