Who can take part
- You are 18 years or older
- You have been diagnosed with heart failure that causes symptoms (like shortness of breath or fatigue)
- You have had a heart failure-related hospital stay or urgent care visit in the last 6 months
- Your kidney function (eGFR) is between 20 and 60 (moderately to severely reduced)
- Your blood NT-proBNP level is above 300 (or above 600 if you have an irregular heart rhythm called atrial fibrillation)
- You are not currently taking a medication known as an MRA (like spironolactone or eplerenone)
