Blood-Guided Chemoradiation for HPV-Positive Throat Cancer

This study tests whether a blood test that looks for circulating HPV DNA (ctDNA) can help doctors decide if you need chemotherapy after surgery for HPV-positive throat cancer. After you have surgery to remove the cancer, you receive radiation (possibly with chemotherapy) — the results of the blood test guide whether or not to add chemo.

Who can take part

  • You must have HPV-positive (p16-positive) oropharyngeal cancer that has spread to neck lymph nodes.
  • You’ve had or will have complete surgical removal of the tumor (using robotic surgery) and a neck dissection.
  • At least one high-risk feature after surgery (like larger tumor size, multiple positive nodes, or cancer in the surgical margins).
  • A blood test (NavDx) must confirm you have detectable HPV DNA in your blood or tumor.
  • You must be in good general health (ECOG 0-2) and have normal kidney, liver, and blood cell counts before starting chemoradiation.
