Who can take part
- You are 50 years old or older.
- You have at least two of these conditions: heart disease, high cardiovascular risk, stroke, type 2 diabetes, osteoarthritis, or overweight/obesity.
- You can walk on your own (no cane or walker needed).
- You are not currently active enough — less than 150 minutes of moderate exercise per week.
- Your medications for these conditions have been stable for at least 3 months.
