- Had your first stroke in the past 3 months (plus 2 weeks)
- Have no or only mild symptoms of depression
- Live alone with a yearly income of $45,000 or less
- Live at home, not in a nursing facility
- Can speak and read English or Spanish
- Have normal or mild memory/thinking problems (score 11 or higher on Mini-MoCA test)
