Who can take part

  • You are 18 years or older.
  • You have been diagnosed with heart failure with reduced ejection fraction (a type of heart failure where the heart doesn't pump blood well).
  • Your ejection fraction (a measure of heart pumping strength) is 40% or less.
  • You have a high level of a heart stress marker called NT-proBNP (over 125 pg/mL) or BNP (35 pg/mL or higher).
  • You are a new user of Jardiance or certain other heart failure medications, meaning you haven't taken them in the past 6 months.
  • You have not taken any SGLT2 inhibitor (a class of diabetes and heart failure drugs) in the past 6 months.
