Who can take part
- You are 18 or older and need a new CRT-D device (a special pacemaker with a defibrillator) for the first time.
- Your heart's pumping ability (LVEF) is 35% or less, measured in the last 6 months after at least 3 months of recommended heart failure medications.
- You have symptoms of heart failure (NYHA class II to IV) despite taking optimal heart failure drugs (such as ACE inhibitors/ARBs/ARNIs, beta blockers, mineralocorticoid receptor antagonists, and SGLT2 inhibitors) for at least 3 months.
- You have a specific electrical heart pattern called left bundle branch block (LBBB) with a wide QRS (>=130 ms in women or >=140 ms in men).
- You agree to use a remote monitoring system (LATITUDE NXT) and can attend follow-up visits at the study site.
- You are not currently in permanent or persistent atrial fibrillation and do not have other conditions requiring a high amount of heart pacing.
