Who can take part

  • You are 40 years old or older.
  • You have been diagnosed with at least two of these conditions: high blood pressure, diabetes, depression, anxiety, COPD, asthma, vision loss, hearing loss, osteoarthritis, or chronic back pain.
  • You are not pregnant or breastfeeding.
  • You do not have severe memory problems or dementia that would make it hard to take part.
  • You are not bedridden or near the end of life.
  • You are not currently in another research study that might interfere.
  • You do not have a severe mental illness (like psychosis or bipolar disorder) other than depression or anxiety.
  • You are willing and able to give written consent and communicate effectively.
