Who can take part

  • You are 40 years old or older
  • You have been diagnosed with at least two chronic conditions from a list including high blood pressure, diabetes, depression, anxiety, COPD, asthma, vision or hearing loss, arthritis, or chronic back pain
  • You attend a Primary Health Centre during the enrollment period
  • You are able to understand and sign a consent form
  • You are not pregnant or breastfeeding
  • You are able to participate in interviews and answer health questions, even with help if needed
