Who can take part
- You had a stroke or high-risk TIA within the past 3 years and your condition is stable
- A recent CT scan (within 3 months) shows 50–99% narrowing of an artery inside your brain
- Your triglyceride levels are elevated (150–499 mg/dL depending on whether you fasted)
- You are 18 years or older and able to give informed consent
- You have not taken pemafibrate or other fibrate drugs in the past 12 weeks
- You do not have severe liver disease, gallstones, pancreatitis history, or dialysis-dependence
