This study tests how a medication called DAPA (an SGLT2 inhibitor) affects the body in people with heart failure (with or without type 2 diabetes), or in some people with type 2 diabetes only. It may help researchers understand who could benefit most and what changes happen in the heart, using specialized heart imaging.

Who can take part
- You’re age 18 or older
- You have either (1) heart failure with an ejection fraction of 40% or less, or (2) type 2 diabetes with normal heart pumping, or (3) be a healthy volunteer
- If you have heart failure: symptoms for at least 2 months and a blood test called NT-proBNP above 125
- If you have type 2 diabetes: it must be confirmed by past tests (HbA1c or fasting glucose) and you’ve been on stable diabetes treatment for at least 12 months
- You must not have taken an SGLT2 inhibitor (like DAPA/other “SGLT2” medicines) within the past 8 weeks (or ever, in the type 2-only group)
- You must meet kidney and blood pressure safety limits and be able to have MRI scans
