This study tests whether a special dietary supplement (ketone esters) can improve exercise ability and heart function in people with a type of heart failure where the heart pumps well but is stiff (HFpEF). You might join if you have stable symptoms, an ejection fraction of 50% or higher, and signs of fluid buildup in the heart.

Who can take part

  • You are 18 years or older.
  • You have stable heart failure symptoms (NYHA class II or III).
  • Your heart's pumping strength (ejection fraction) is 50% or higher.
  • You have signs of increased pressure or fluid in your heart (like an enlarged upper chamber, or need water pills daily, or high BNP levels).
  • You are not on a strict ketogenic diet, don't have cirrhosis or heavy alcohol use, and don't have other conditions that limit exercise (like severe lung disease, uncontrolled high blood pressure, or recent heart attack).
  • You don't have Type 1 diabetes and haven't started a GLP-1 medication within the last 6 months.
