Who can take part
- Age 18–75 and able to understand and consent to the study
- Non-small cell lung cancer (NSCLC) with EGFR mutation (L858R or exon 19 deletion) that has spread to the lining around the brain after third-generation EGFR inhibitor treatment
- Positive cerebrospinal fluid (CSF) test confirming leptomeningeal metastasis, OR clinical diagnosis based on symptoms and brain/spine imaging
- Already had surgery to place an Ommaya reservoir (a small catheter in the brain) and it's working without complications
- Blood counts, kidney, and liver function within acceptable ranges; able to swallow pills; stable neurological symptoms (no need for increased steroids in past 1 week)
- No recent brain/spinal radiation (within 6 months) or major surgery (within 4 weeks); no serious heart disease, uncontrolled high blood pressure, or active HIV; not pregnant or breastfeeding
